I acknowledge that I have voluntarily agreed to engage and otherwise participate in the Cia Paulista Australia Workshop being held by Cia Paulista Australia at Victoria University - Footscray Park on the 25th of May, 2019. I am aware that the activities constitute a potentially dangerous activity in which injury may occur. I am voluntarily participating in these activities with full knowledge of the danger involved and hereby agree to accept all risks of injury or death when so engaged.
I hereby release Cia Paulista Australia, it's instructor and other authorized agents, employees and any of it's affiliated organizations from all the actions, claims and demands that I, my assignees, heirs, guardians and legal representative now have or may hereafter have for injury, damage or loss resulting from my participation in this workshop as organized by Cia Paulista Australia and it's employees and appointed instructors.
I have carefully read this agreement and fully understand it's contents. I am aware that this is a release of liability and that it forms a contract between myself and Cia Paulista Australia, it's agents, instructors and employees and I agree to it of my free will.
